            This week’s discussion mentions the importance of realistic goal setting when implementing strategies. In addition, goal setting is crucial so the parties implementing strategies know what they are working toward. Someone with some great organization skills should be in charge of keeping track of the goal itself and the progress that is being made. The SMART methodology is a great way to help one simplify the goals at hand. The acronym “SMART” stands for: specific, measurable, attainable, relevant and timely (Career Wise, 2018, P.1).

            The first step of SMART is “specific.” This step helps define exactly what is wanting to be accomplished. Second, is “measurable.” This phase is meant for breaking down trackable elements to record them at each stage. Thirdly, SMART presents “attainable.” In essence, this step holds one accountable for creating goals that are realistic to achieve. The fourth step is “relevant.” The relevant step is to recognize WHY one is setting the goal. Essentially, this step keeps you on track to remember the issue at hand. The final step of SMART is “timely.” Timely is a crucial step to maintain a realistic timeframe for the goal to be met. (Career Wise, 2018, P.1)

            Whenever a project is getting started, it is pertinent to lay out the objective of the project in its entirety. Using the SMART method basically organizes your work for you, you just have to fill in the blanks. Defining a specific issue of a targeting community gives you an objective; make it better!

            Implementing strategies that is difficult to do if there is not a solid foundation and clear goals and expectations to accomplish the strategy. The SMART tool makes it possible and clearly defines each step that is needed. S-specific and goes into the 6 W’s that need to be answered in order to ensure the mission is accomplished. M-measurable, this is where having a foundation and measuring the success of the strategy is considered. A-achievable and is intended to motivate yourself and those involved. R- relevant to the mission statement and goals. T-time tables to ensure that the strategy is adhering to a possible schedule (Eby, 2019). Having this tool assists with defining the specific goals and objectives.

            Having an end point in mind is important; however, if there is no map to get you there the objective will be lost. Utilizing the SMART tool and being as detailed as possible makes that a possibility. Within the 6 W’s there are who, what, when, where, which, and why. These W’s give the specificity needed in order to be closer to accomplishing the goal (Eby, 2019). It also allows for the goal to be tailored to a specific community and ensures it aligns with their culture. 

            By tailoring the SMART tool you can look at the disparities of an itemized group by the following traits shared values, traditions, customs, and history, as well as beliefs and practices of the population (Torres-Ruiz et al., 2018). Tailoring it ensures that there is more consistency with the information needed and that the objective is not overlooked. By keeping a clear objective it keeps those involved motivated and prevents any possible confusion.

Preparing Your Creating a Logic Model Paper you were asked to conduct an assessment of a health topic in a community of of your choice using the PRECEDE portion of the PRECEDE-PROCEED model. This week, you will draw on that assessment to develop a culturally-tailored program intervention and describe your program in the context of a logic model to address your health topic of concern. This paper will comprise one part of the final assignment (Grant Proposal paper) for this course. Once your logic model paper is graded, you will be asked to revise the paper according to instructor feedback and incorporate the sections into your final assignment .

For the purpose of this paper:

You will apply your awareness of cultural values and practices in the development of a program intervention. Your intervention should be 12 months in length and have a budget of $100,000 to implement and staff your program. Please follow the steps below.

1. Create a hypothetical organization that has a history working with your identified priority population in your focus community. Feel free to model your organizational description after an actual organization that works with your identified priority population or in your target community (1 page).
   1. Describe your organization. Be concise, specific and compelling. Explain what is unique about your organization and why you are well-situated to work with your priority population and to address your identified issue. Use the description to build credibility for your organization.
   2. Identify at least one partner agency that you will be collaborating with. Indicate who they are and the purpose of your collaboration. Collaborations come in several different forms. Be sure to explain how this collaboration will work and the role(s) of each organization.
2. Describe the health problem within the target population. Incorporate your findings from your Module 3 assignment.
   1. Present the existence of the health problem. Provide a description of the size of the public health problem at national, state, and local levels. At the local level, make sure to explain how it relates to your target population.
   2. Describe causes of the health problem. Identify the various factors that contribute to the existence of the health problem and discuss this relationship in terms of a causal theory.
   3. Zero in on specific factor(s) that your organization will address and present a clear, concise, well-supported statement of the problem to be overcome. Make a connection between the issue and your organization. Explain consequences if health problem is not addressed.

4. Describe your plan for implementation.
   1. Describe your organizational plan. Specify the resources (inputs and capacity) that your organization will need to implement and sustain your program intervention. (1 page)
   2. Describe your service utilization plan. This includes a description of culturally appropriate intervention activities that will be developed and implemented (with available resources) to achieve your program objectives. Describe why you have chosen these activities and how they are culturally sensitive and appropriate. (1-2 pages)
   3. Develop a logic model visual that clearly connects inputs to activities and activities to outputs. For each action step, list projected completion dates and person(s) responsible.

1. The Creating a Logic Model Paper is worth 280 points and will be graded on quality of information, use of citations, use of standard English grammar, sentence structure, and overall organization based on the required components as summarized in the directions and grading criteria/rubric.
2. Create your paper using Microsoft Word, which is the required format for all Chamberlain College documents.
