This assessment provides an opportunity for you to present Mrs. Snyder’s case to stakeholders.

  Preparation
  For your final assessment, you will present Mrs. Snyder’s case to the health care team, including all internal and external stakeholders and key family members. The purpose of the presentation is to ensure that everyone connected with Mrs. Snyder’s case is well informed and that they have a common understanding of her care to date and of plans for providing the best possible patient-centered care.
  To prepare for the presentation, you are encouraged to review the care plan and the transitional care plan that you developed for Mrs. Snyder in Assessments 2 and 3.

  Presentation Software
  You may use Microsoft PowerPoint or any other suitable presentation software to create your slides. Use the speaker’s notes section of each slide to develop your talking points and cite your sources, as appropriate. If you need help designing your presentation, you are encouraged to review the various presentation resources provided.
  Inclusion of audio voiceover or video is optional. Required information should be included in the slides and speaker notes.

In many ways, personal plans are similar to strategic plans. We have goals, objectives, a strategy, and an action plan. For your final assignment, you are to develop your own 5-year plan for your career in healthcare. In it, list at least 2 goals and 3 objectives for each goal. Briefly describe your overall plan and the specific action steps you will undertake to implement your plan. Be sure to include a timeline. This paper should be a minimum of 2 pages but if you want to write more, please feel free to do so – it is your plan! Be sure to format your paper per APA standards and be sure to use in-text citations where necessary. Also, be sure to include a reference page when required.

Your company has just hired your foreign friend to work in a middle-management position. Since you have lived in the United States for many years, your friend believes that you understand job coaching for a traditional American company.
