·         Review the information in this week’s Learning Resources regarding the stages of group formation, problematic roles individuals play in groups, and strategies for facilitating and maintaining positive group collaboration. In particular, review Learning Exercise 19.12 on page 464 of the course text.

·         Reflect on various groups with which you have been or are currently involved. Select one specific group to analyze for the purposes of this Discussion. Identify the purpose or task that the group is or was meant to perform.

·         Consider the four stages of group formation (forming, storming, norming, and performing). How would you describe the progression between stages? Is there a stage in which you believe your group is or was “stuck”?

·         Consider the task or group-building role you normally play in a group setting. How could you apply the information from the Learning Resources to improve your group participation and facilitation, as well as the functioning of the group as a whole?

·         In addition, think about which individuals within your group (including yourself) may fall into problematic roles such as the Dominator, the Aggressor, or the Blocker. It has been two months since the disaster and you have just been informed that these things are what caused the blowout that lead to the explosion on the Deepwater Horizon. Use your knowledge to construct a Preliminary Hazard List (PHL) for the items that you feel can lead to these issues reoccurring. The video entitled, “BP Deepwater Horizon Accident Investigation Report” will help to increase your knowledge on how the drilling operation works. Tell your boss (for the purposes of all the case studies in this class your boss is your Instructor) in a one to two page document what the hazards associated with this drilling operation are. This report should include an actual PHL and a short narrative talking about the items in the PHL.

In the next module, you will use this PHL to construct a Preliminary Hazard Analysis (PHA), so be sure to get a thorough foundation here. There are no right or wrong answers to this as it is the perception of the safety person/team doing the analysis. If you feel you need to research this accident further, you are free to use any credible sources to get a better feel for what happened. Just remember, if you use something other than the sources above, be sure to give proper credit in your report.

· The facility had three safety events involving a patient receiving blood transfusions of the wrong blood type. All three patients died. Two (2) patients received their transfusions on a medicine unit and the other patient received his transfusion on a surgical unit. After a root cause analysis, it was found that transfusion orders for each patient were meant for other patients. Additionally, it was found that in two of the cases there were blood specimen labeling errors. 

• In the six (6) months before the deaths of these three patients, the facility had received ten (10) patient safety event reports (JPSR) related to blood labeling errors, but all were caught before a patient was harmed. When staff were interviewed about the event, many reported that communication between clinical staff has always been an issue. Several of the physicians interviewed stated that it is not uncommon for the Laboratory to make errors because nothing is ever done to correct them. The Medical Center Director has met with the staff members. They all report feeling really bad that this happened. They believe in the care they provide and only want to do a good job.
