Only two question to be answered. At least 50 words per question and needs to be sited properly using scholarly references. This is not due until Saturday which will is the time allotted to get this completed. However, I do not suspect it will take more than an hour to complete. Thank you in advance.

Question 1: Why is it important to consider the first session the beginning of discharge planning? Why is it important to prepare an accurate and consistent discharge summary? Be sure to include ethical considerations and involvement of the client.

Question 2:How would you prepare a client for discharge? What information should be discussed in sessions leading up to termination?
